Activation Form

The activation form must be filled out and returned to us prior to using the unit.  Save a copy of the form to your computer to keep for future reference.  The form may be filled out on the computer.  There are buttons to either Print or Email the form.  If printing and FAXing the form, FAX to 301-576-5212.
